Form A: Declaration of residence on the island of Ireland by EU/EEA & Swiss national

About Form A
Use Form A if you wish to apply for a passport for your child, if your child was born on the island of Ireland (including Northern Ireland) on or after 1 January 2005.

By completing this form you declare that you were legally resident on the island of Ireland (including Northern Ireland) for 3-out-of-4 years immediately before the birth of your child.

To make the declaration, download and complete Form A.

Passport Application
To apply for your child’s passport, send the Declaration of Residence and passport application form to the Department of Foreign Affairs for processing. (Do not send to ISD Citizenship Division).

Form C: Declaration of residence on the island of Ireland by non EU/EEA & Swiss nationals

About Form C
Use Form C if your child was born on the island of Ireland (including Northern Ireland) on or after 1 January 2005 and you wish to apply for a passport on behalf of your child.

By completing this form you declare that you were legally resident in Northern Ireland for 3-out-of-4 years immediately before the birth of your child.

Download and complete Form C and send it to the Citizenship Division (at the address below) for processing.

If your declaration is approved, your child will be issued with a Certificate of Nationality by post.
